MVSF Forms Update

The Portal is used to perform all licensing functions.

• New license applications, license renewals, amendments such as address, management, and contacts, etc. 
• There are no paper versions or alternatives to The Portal
• Instructions are in The Portal User Guide.
• A Keystone Login is used to access The Portal
• Use your existing Keystone Login, or create one at https://keystonelogin.pa.gov/

Sales Finance:

Use this form to secure a bond as required for sales finance licensure.

Collector-Repossessor:

Collector-Repossessor Bond (PDF)
Use this form to secure a bond as required for collector-repossessor licensure.

Criminal Record Check and Fingerprint Check (PDF)
These forms must be completed along with license applications.  It is also required for all owners, partners, corporations, managers, and officers.
